You will receive information on what forms are due at this time for spring sports

 

ALL STUDENTS MUST SUBMIT PAPERWORK EACH SEASON EACH SPORT

·  If you have participated in a fall or winter sport and have a physical on file in the CDMS health office and it has not or is not due to expire please submit a Health History Questionnaire Update Permission Form for the spring sport you are interested in. 

·  If there is no physical or sport packet submitted for the 2023-2024 school year  please submit the entire sport packet 

·  Physicals are good for 365 days

 

Remember ALL forms must be reviewed and cleared BEFORE students are permitted to practice or try out.

Sport Sign Ups:

  •  FALL --- Mail hard copy sport packet documents or drop off at school completed documents/sport packet
  • WINTER and SPRING--- STUDENTS please see Mrs. Heyer, CDMS Nurse, to sign up for winter or spring sports and receive guidance on forms required for the specific sport/ season

  • All students MUST submit forms EACH SEASON for EACH SPORT. Please see the "Sports Packet Guidelines"
  • Practice and Try-Outs are anticipated to start for students the first full week of school in September for those who have submitted documents for review and have received permission for participation
  • Please see site under ATHLETICS for more information on sports from coaches or coaches for additional information
  • 2023-2024 Sports Forms - available under sport forms - mail hard copies or drop off to school. Please do not email documents unless discussed with Mrs. Heyer.
  • 6th grade student required vaccinations (Tdap + Meningococcal) ARE part of the sport physical

IMMUNIZATIONS - attention incoming 6th grade parents

  • New Jersey State regulations require students entering 6th grade to provide proof of Diphtheria and Tetanus Toxoids and Pertussis vaccine (Tdap) and Meningococcal vaccine.
  • Documentation is required by the NJ Health Dept. to be in place by the start of 6th grade.
  • In the event immunizations are not received, a note from your medical provider's office of a scheduled appointment for the above mentioned immunizations will be required to maintain compliance with follow up documentation. Student records are inspected annually by the Health Department for compliance.
  • It is recommended that you keep a copy for your own medical records.
  • Please mail, drop off or send to school with your child by the first day of school. Email is not recommended during summer months.

 

COVID-19 VACCINATIONS

  • If your child has received vaccinations and/ or booster for Covid 19 - you may submit a copy of their immunization card for immunization update on their student health record. This record follows them from pre-K - gr 12.

MEDICATION

  • In the event that your child will need medication during school hours it is necessary that all forms be filled out properly and returned. New forms are required each school year.
  • Students MAY NOT carry medication with them in school  [prescription or over the counter] unless it is their EMERGENCY medication [Epi-Pen or Asthma inhaler] with an Allergy Action Plan or Asthma Action Plan on file in the CDMS Health Office for the current school year signed by a health care provider and parent. 
  • HEALTH OFFICE FORMS are available on the CDMS nurse page.
  • Parents must bring in all medications and signed forms to the health office. All medication (prescription and over the counter medication) must be in the original labeled containers/packages. Medications cannot be dispensed without a parent/guardian signature, physician signature office stamp, and original packaging.

 

SPORT PHYSICALS

  • School sports are offered at Cedar Drive Middle School to students in grades 6 - 7- 8.
  • Participation in school sports requires a complete physical exam prior to participation per New Jersey Department of Education Regulations. The only accepted NJ forms can be downloaded from the CDMS Nurse's page along with guidelines.
  • All forms must be received and reviewed by the school nurse and or school physician BEFORE a student can participate in school sports.
  • PROOF OF 6TH GRADE IMMUNIZATIONS OR A LETTER FROM YOUR HEALTH CARE PROVIDER OF SCHEDULED DATE TO RECEIVE Tdap and/or Meningococcal VACCINATIONS IS PART OF THE 6TH GRADE SPORT PHYSICAL DOCUMENTS for incoming 6th grade students.
  • Physicals are good for 365 days. Current physicals on file in the health office from the previous school year do not need to be resubmitted.
  • See procedure for sport forms located under sport forms for additional information.
  • Additional forms are required for each sport season.
  • Students who have not received clearance may not participate in any practice or tryout.
  • Please adhere to all deadlines and guidelines to assure that your child is able to participate at the start of the sport season.

 

HELPFUL HINTS 

  • Review emergency information and contacts is advised in preparation for the new school year
  • Update health concerns, allergies, and or medical concerns with the school nurse in September and as needed
  • Schedule appointments early and have your medical care provider fill out forms in the summer in preparation for fall sports ~~~ Offices are busy early September and may require additional time to process forms all sport - participation and or tryouts generally begin the first full week of school in September
  • Please keep copies for your own records in preparation for future reference during middle school years and moving forward to high school
  • Medication Plans and Permission forms for the 2023 - 2024 school year are available under Health Office Forms.
  • In the event that your child will need medication in September for the 2023 - 2024 school year, it is necessary that all forms be filled out properly, signed, office stamped and returned at the start of the school year
  • Students that carry their own emergency medications (Epinephrine or inhalers) MUST have Action Plans on file in the Health Office for the current school year
  • Medication MUST be presented in the original container with a label or prescription label. This will facilitate the dispensing of medication at the start of the school year.
