New Incoming Students

Students new to the Colts Neck Township School District will need to complete the required forms and provide the records requested below. 

 

Step 1: Health History Revised

(Parent can complete this form.)

 

Step 2: Pre-Entry Physical Form

(Doctor to complete, sign and stamp.  All physicals must be within a year of starting school.)

 

Step 3: Immunization Guidelines

(Must be obtained from physician.)
